1. NOTE: The Dads Club Swim Start Staff is trained to place your child into an age/ability appropriate class. After submitting your form you will receive a phone call within 3 business days to place your child into a class. Please note that we take registrations in the order that they are received.
2. There is a comments box towards the end of the registration form. Please use this box to indicate days and times available, start date and any special needs that your swimmer might have.
3. Submitting your registration form indicates that you have read and accept all of the    DADS CLUB SWIM START POLICIES AND PROCECDURES
